Home and Community Based Waiver Program Services
- Homemaker- general light household activities, laundry and meal preparation
- Respite Care - provides patients (who require daily 24-hour care) and their caregiver(s) short-term relief so the person who provides the care may run errands, etc.
- Personal Care- basic personal care grooming and activities of daily living.
- Minor Home Adaptations - small changes or additions made to the patient's home. This will allow the recipient to remain in their current living environment. (Bathtub rails, commode rails, etc.)
- Attendant Care - Provides care for someone who has a caregiver who works full-time outside the home. This caregiver is not required to live in the recipient's home (up to 45 hours per week).
- Model II Ventilator Services - patients who require the use of a home ventilator machine can qualify for up to 18 hours of nursing services per day to assist the caregiver.
